Step 1 – Obtain a physical from your physician (Physical Form)

Step 2 – Complete an online profile on athleticclearance.com, where you will be prompted to upload the physical (Athletic Clearance Tutorial)

Step 3 – you will receive a confirmation email from CIF Home Campus

Physical Form -  This form is required for all LAHS student who will be participating in any athletic sport during the school year.
